Expert Advice about COVID-19 Vaccines and Safety for Kids

After a long and anxious wait, COVID-19 vaccines are now available for kids age 5 to 11. Getting young kids vaccinated is critical for protecting our communities, since this is a large age group, and taking this step will give millions of parents peace of mind. 

Paul Lewis, MD, a Kaiser Permanente pediatrician in Portland, Oregon, who specializes in infections in kids, says that as winter and the holidays approach, this is a good time to get kids vaccinated. He suggests using the COVID-19 vaccine authorization as an opportunity to get kids caught up on other vaccinations as well, since many people have fallen behind on regular appointments during the last 18 months. He also suggests that all children over 6 months old get their flu shot this year. Fortunately, he says, it’s no problem to get both shots at the same time.

We know parents, caregivers, and others may have questions about the safety of the pediatric COVID-19 vaccine. In this Q&A, Dr. Lewis answers common questions about it.

How well does the COVID-19 vaccine work for kids?

For kids under 12, the COVID-19 vaccine is incredibly effective. A recent study, affirmed by the Food and Drug Administration, that included more than 3,000 kids who got the pediatric dose of the vaccine found that it was more than 90% effective at preventing infection with symptoms.

 

How does the vaccine for children 5 to 11 differ from the vaccine for people 12 and older?

The pediatric vaccine is identical to the vaccine that’s used for older teenagers and adults, but the dose is one-third of the adult dose. That dosage was chosen because it was just as effective at providing protective antibodies but had a much lower rate of side effects.

What are the most common side effects?

Of the thousands of kids who were part of that recent study, zero had serious adverse events. Some kids get sore arms, redness or swelling, and a smaller percentage might get fatigue, muscle aches or fever. But they’re getting side effects at lower rates than teenagers and adults.

 

What is the risk of myocarditis?

Myocarditis, or inflammation of the heart, has occurred in young men who’ve been vaccinated but is pretty rare, with a handful of cases per million vaccination doses. Many cardiologists think that, since this is generally more common in teenagers than school-age kids, it’ll be less common after vaccination as well. 

If my child has underlying health conditions, should I go ahead with vaccination?

Studies on this are ongoing. We know that older adolescents and adults with immunocompromised conditions or on immunosuppressive medications respond less well to vaccination, but they do respond. We also know that if they get COVID-19, they’re more likely to have worse outcomes, including needing hospitalization and intensive care. If someone has an underlying condition, such as an immunosuppressive condition, diabetes, heart disease or lung disease, we recommend getting an age-appropriate vaccine.

If my child is used to wearing a mask, can’t I just rely on that for prevention?

Social distancing and wearing masks are important layers of protection, but they are hard to continue day after day, especially for kids at recess or at lunch. So the vaccine is a big, thick layer of protection to help with all those other measures, and people should use them together. 

How should I prepare my child for a vaccination appointment?

I’ve never met a kid who wanted to get a shot, but kids are resilient. You can help by explaining that you’ve been vaccinated and, while it does hurt, the pain goes away quickly and you’re there for them. At Kaiser Permanente vaccination sites, there will be people who are used to dealing with children and who know that there is a lot of emotion and fear. Kaiser Permanente staff and parents can work together to make it a minor experience for kids.

We Asked an Expert Your Car Seat Questions—Here Are Their Responses

Between finding the right car seat, installing it correctly and making sure it fits, keeping your little one safe on the road is a big task. We teamed up with Nuna, the creators of parent-approved car seats like the Nuna PIPA lite rx, and car seat expert Bob Wall, a leader in the Child Passenger Safety field for over 30 years, to answer your questions.

First up, “How do I pick a car seat?” Here’s what Wall had to say:

The short answer to this question is, read the specifics of the seat you’re looking at and match it to your child’s weight and height and their developmental level. There is a car seat out there to fit all sizes of children, you just need to match the seat to your child.

This sounds easy, but there are several other considerations.

  • How easy is the car seat to use?
  • Does it fit in your vehicle?
  • Does it fit your personal lifestyle and family’s needs?
  • Does it accommodate your child’s needs?

"How Do I Know If My Car Seat Is the Right Size for My Child?"

The different types of car seats are designed to target specific weights, sizes, and ages of children. The primary categories are infant-only, convertible, harness to booster (combination) and booster. Some of these categories have child-size and direction overlaps, and choosing the correct seat could be confusing.

Infant-only seats start at 4 or 5 lbs and are rear-facing only. Convertible seats also start at approximately 4 or 5 lbs and can be forward-facing after the child uses it in the rear-facing mode until its maximum weight and height, and the child is at least two years of age. Harness to booster (combination) seats are forward-facing only seats that can be used right after the rear-facing seat has been maxed out by the child's height or weight. The child would use this seat forward-facing with the harness until the child outgrows the seat and can be placed in a booster. Booster seats vary in size but most start at 40 lbs and an age minimum—typically age four or five). The booster would be used with the vehicle’s seat belt to ensure the child fits the belt correctly.

"At What Age/Weight Should I Change My Child’s Car Seat from Rear-Facing to Front-Facing?"

The American Academy of Pediatrics and the National Highway Traffic Safety Administration recommend that infants rear face as long as possible until they reach the highest weight or height allowed by their car seat manufacturer. Most convertible car seats have limits that will permit children to ride rear-facing for two years or more. Having your child rear-face as long as possible is the safest way for them to ride. A rear-facing car seat will support the child’s extremely vulnerable head, neck, and spine. When children ride forward-facing, their heads—which are disproportionately larger and heavier for babies and toddlers—can jolt forward, possibly resulting in spine and head injuries.

Remember: When your child has outgrown their rear-facing seat, secure them in a forward-facing car seat with a harness for as long as possible, up to the highest weight or height limit allowed by the car seat manufacturer.

"If I Was in a Car Accident, How Do I Know If My Child’s Car Seat Is Still Okay to Use?"

If you have been involved in a minor crash then we recommend you follow the NHTSA guidelines: NHTSA recommends that car seats be replaced following a moderate or severe crash in order to ensure a continued high level of crash protection for child passengers.

Car seats do not automatically need to be replaced following a minor crash.

A minor crash is one in which ALL of the following apply:
• The vehicle was able to be driven away from the crash site.
• The vehicle door nearest the car seat was not damaged.
• None of the passengers in the vehicle sustained any injuries in the crash.
• If the vehicle has airbags, the airbags did not deploy during the crash; and
• There is no visible damage to the car seat.

NEVER use a car seat that has been involved in a moderate to severe crash. Always follow the manufacturer's instructions.

"What’s the Safest Place to Put the Car Seat? Behind Driver, Middle, behind Passenger?"

All seats in the rear are safe if the child seat is installed correctly and you can install it correctly on every trip. The discernment of “center is safest” originates from the center location being the farthest point from any intrusion into the passenger seating area during a crash. Generally, the safest location in the vehicle is the center rear of the vehicle. However, this is not always the case, the center seat may not be compatible with the seat you are using or there could be a practicality issue depending on your situation. An example would be: If you want the youngest in the center because it's “the safest”, but to get the baby in the center you have to lift the infant carrier over the other seat or child. Or it could be as simple as you are unable to install it as well in the center as you can on the sides. The center seating location is only the safest if you can use it correctly every time the child is in the vehicle. To say the center rear is the “safest” is not saying the other seating positions in the back are not safe—they are.

"What If My Child Is in the Height Range to Switch to a Booster Seat but Not Out of the Weight Range?"

Boosters main job is to ensure the child can sit securely using the lap and shoulder belts in the vehicle by lifting the child up so the belt fits correctly. These are called belt-positioning booster seats, and to ensure your child is meeting the criteria to move to a seat belt, you should follow the 5-Step Seat Belt Fit Test. 

1. Shoulder belt crosses between the neck and shoulder.

  • If the shoulder belt is too close to the neck, kids can be tempted to put it behind their back for comfort. A shoulder belt that sits off the shoulder can slip off during a crash, reducing its ability to protect.

2. Lower back is against the vehicle seat.

  • If the child is sitting with their bottom forward to allow their legs to go over the edge of the seat to feel comfortable, a gap is created between their back and the seat. This will cause the seat belt to ride up out of position onto their belly. It can also introduce slack in the seat belt, allowing the child to move forward more during a crash. Both of these can cause increased injury in a crash.

3. Lap belt stays on the upper thighs across the hip bones.

  • If the lap portion of the belt is across the soft tissue of the abdomen (like will happen if their back isn’t against the vehicle seat), it can damage internal organs in a crash.

4. The knees bend at the end of the seat.

  • Kids will scoot their bottom forward to let their knees bend comfortably, increasing their risk of injury because the seat belt rides up off of their hips and onto the soft part of their belly. They need to be tall enough to have their knees comfortably bend at the edge of the seat.

5. The child can ride like this for the entire ride.

  • We don’t expect the child to be perfectly still while riding in the car. And their movement or readjustment to stay comfortable cannot lead to the seat belt getting out of position. When children get uncomfortable, they tend to slouch, lean to one side or put the shoulder belt behind them. When the seat belt is out of position, it cannot properly protect the child during a crash.

INTERNET: Tinybeans and Paramount Pictures are not responsible for electronic transmission errors resulting in omission, interruption, deletion, defect, delay in operations or transmission or alterations of entry materials, or for technical, network, telephone equipment, electronic, computer, hardware or software malfunctions or limitations of any kind, or inaccurate transmissions of or failure to receive entry information by Tinybeans on account of technical problems or traffic congestion on the Internet or at any website or any combination thereof. If for any reason the promotions are not capable of running as planned for any reason Tinybeans reserves the right in its sole discretion, to cancel, terminate, modify or suspend the contest and select the winners by random drawing from among all eligible entries received from all methods combined up to the point of the action taken by the Tinybeans.

 

CONTEST ENTRY REQUIREMENTS & CONDITIONS: By submitting a contest Entry, contest Entrant warrants and represents that: (a) contest Entry does not infringe upon the copyrights, trademarks, rights of privacy, publicity or other intellectual property or other legal or moral rights of any person or entity; (b) contest Entrant owns all rights to the contest Entry, including without limitation, copyrights, and has received prior written permission from a third party if any Submission was photographed or videoed by someone other than the contest Entrant him/herself; and (c) contest Entrant will indemnify and hold harmless the Sponsor, Administrator and related entities, agents and assigns from any claims and damages (including reasonable attorneys’ fees) arising from or relating to the breach or alleged breach of your representations and warranties herein, the contest Entry, or contest Entrant’s conduct in creating the contest Entry, the acceptance or use of any prize or otherwise in connection with this contest.

 

PHOTO SUBMISSIONS/RELEASES: If the photograph or video submission contains any material or elements that are not owned by the entrant and/or which are subject to the rights of third parties, and/or if any persons appear in the photograph or video, the entrant is responsible for obtaining, prior to submission of the photograph or video, any and all releases and consents necessary to permit the exhibition and use of the photograph or video in the manner set forth in these Official Rules without additional compensation. By participating in the contest, participants give Sponsor and Tinybeans the right to share participant’s images, videos and text. Participants acknowledge and agree that the Sponsor may at any time use any entry provided by you for internal business purposes and promotional external media (placement on the sponsor website, inclusion in sponsor social media or email blasts for example) that is directly related to the photo contest.

 

LIMITATION OF LIABILITY: By entering this contest, all entrants agree to release, discharge, and hold harmless Sponsor from any claims, losses, and damages arising out of their participation in this contest or any contest-related activities and the acceptance and use, misuse, or possession of any prize awarded hereunder. Tinybeans assumes no responsibility for any error, omission, interruption, deletion, defect, or delay in operation or transmission; communications line failure; theft or destruction of or unauthorized access to contest entries or entry forms; or alteration of entries or entry forms. Sponsor is not responsible for any problems with or technical malfunction of any telephone network or lines, computer online systems, servers or providers, computer equipment, software, failure of any entry to be received on account of technical problems or traffic congestion on the Internet or at any website, human errors of any kind, or any combination thereof, including any injury or damage to entrants’ or any other persons’ computers related to or resulting from participation, uploading or downloading of any materials related to in this contest.

 

RIGHT TO CANCEL OR SUSPEND contest: If for any reason the contest is not capable of running as planned, due to infection by computer virus, bugs, worms, trojan horses, denial of service attacks, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Sponsor that corrupt or affect the administration, security, fairness, integrity, or proper conduct of this contest, Sponsor reserves the right, at its sole discretion, to disqualify any individual(s) who tamper with the entry process, and/or to cancel, terminate, modify, or suspend the contest.

AMENDMENT: Any and all rules and regulations stated within, may be amended for a specific contest to be in compliance with the rules and regulations of a specific Sponsor. Any amendment of rules and regulations will be stated in the contest post.
